How Medicare pays for hospice, in plain words
- Who qualifies. The person has Medicare Part A. A doctor certifies that, if the illness runs its usual course, life expectancy is six months or less. The person chooses comfort care instead of treatment to cure the illness, and signs a form choosing hospice.
- What Medicare covers. For care related to the terminal illness: nurse and aide visits, social work and spiritual care, medical equipment like a hospital bed or wheelchair, medical supplies, medicines for pain and symptoms, short inpatient stays when symptoms can’t be managed at home, short respite stays so caregivers can rest, and grief support for the family.
- What families pay. Usually nothing for covered hospice care. There can be a copay of up to $5 per prescription for pain and symptom medicines, and 5% of the Medicare-approved amount for inpatient respite care.
- What Medicare doesn’t cover. Room and board (at home, in assisted living or in a nursing home), round-the-clock private caregivers, and treatment meant to cure the terminal illness.
- How long it lasts. Two 90-day periods, then an unlimited number of 60-day periods, as long as a doctor recertifies that the person still qualifies.
- You can change your mind. A person can leave hospice at any time to go back to treatment, and can choose hospice again later if they still qualify. They can keep their own doctor.
- Other coverage. If you have a Medicare Advantage plan, Original Medicare still pays for hospice. Regular Medicare keeps covering health problems that aren’t related to the terminal illness.
How to get started (families)
- Call (714) 844-7777. You can call us yourself; you don’t need to wait for a doctor or hospital to bring up hospice. Calling is a conversation, not a commitment.
- We answer your questions, explain how the Medicare hospice benefit works, and talk through whether hospice may fit.
- With your permission, we contact your loved one’s doctor for records and an order for a hospice evaluation. A doctor must certify eligibility before hospice begins.
- A member of our clinical team visits you at home or at the facility, at a time we agree on together.
- Your family decides. If your loved one qualifies and chooses hospice, we plan care with you.
How to refer a patient (doctors, hospitals and case managers)
- Call (714) 844-7777. Open 24 hours, 7 days a week. A live person answers our phone. A phone call is enough to start.
- Ask us for fax and secure transmission instructions. Please never send patient records through a website form or regular email.
- Helpful to send: the physician order, the hospice diagnosis and recent notes showing decline, the medication list, the face sheet and insurance information, code status or POLST, the care setting, and the decision-maker’s contact and preferred language.
- We coordinate with the attending physician, schedule an evaluation with the patient and family, and, with appropriate authorization, keep you updated.
