Care where your loved one lives
Hospice care comes to a private home, a board and care home, assisted living, or a skilled nursing facility. Your loved one’s team includes nurses, hospice aides, a medical social worker, a chaplain, and trained volunteers. They work with your loved one’s own doctor and our hospice medical director.
The four Medicare levels of hospice care
Medicare defines four levels, and the team and doctor choose the one that fits, which can change from day to day. Routine home care is the most common: scheduled visits, pain and symptom management, help with personal care, and a nurse a phone call away. Continuous home care provides mostly nursing care at home for extended hours during a short crisis, such as pain or breathing that is hard to control, so your loved one can stay home when that level is appropriate. General inpatient care is used when symptoms cannot be managed at home. Respite care gives family caregivers a break while their loved one stays, on average 1 to 5 days, in a licensed skilled nursing facility.
Who qualifies under Medicare?
According to Medicare.gov, a person with Medicare Part A qualifies when the hospice doctor and their regular doctor (if they have one) certify a terminal illness with a life expectancy of 6 months or less if the illness runs its normal course, and the person chooses comfort care instead of curative treatment. For patients who qualify, the benefit covers care related to the terminal illness, including medications for pain and symptoms and medical equipment. We can also check Medicare Advantage coverage.
