When a client’s health is declining, you’re often the first person the family calls. They may be a fiduciary’s client, a trust beneficiary, a care-management family or a resident in your community. This page explains how Maxcare Hospice works with you: when it can make sense to ask us about hospice, how to start, what information helps, and what you can expect to hear back from us.
Who this page is for
- Professional fiduciaries, conservators and trustees
- Elder-law and estate-planning attorneys
- Geriatric care managers (aging life care professionals)
- Assisted living, memory care, board and care, and independent living staff
Where we care
Our office is in Westminster. Our team serves Orange County and nearby communities, visiting patients in private homes, assisted living and memory care communities, board and care homes, and skilled nursing facilities. To confirm coverage for a client’s address, please call our 24/7 line at (714) 844-7777. See where we care.
When it may make sense to ask about a hospice evaluation
You don’t have to decide whether a client qualifies. That’s a clinical decision, and a physician has to certify it. These are common, appropriate reasons to call us:
- The client’s doctor has mentioned hospice, comfort care or a limited prognosis.
- The client has had repeated hospital stays or ER visits for the same serious illness.
- You or the family are noticing steady decline: more time in bed, weight loss, needing more help with daily care, or a dementia that is progressing.
- The client, or the person with legal authority to decide for them, wants care focused on comfort and wants to stay where they live.
- Family or facility caregivers are struggling with symptoms, and nobody has explained what hospice would cover.
What hospice is not: it isn’t round-the-clock private-duty caregiving, and it doesn’t replace the client’s legal or financial decision-makers. Hospice decisions belong to the patient or their authorized representative.
How to start
- Call our 24/7 line at (714) 844-7777. You can also suggest that the family call us themselves: families can ask us about hospice directly.
- We talk with the decision-maker. We explain hospice and the Medicare hospice benefit in English or Vietnamese, and we check whether the client may be eligible.
- We work with the client’s doctor. A physician must certify eligibility. With permission, we request records and an order for an evaluation.
- We arrange an evaluation visit at a time that works for the client and family, wherever the client lives.
Information that helps
You don’t need all of this to start. Please share clinical details by phone or through the secure channel we give you, never by website form or regular email.
- Your name, organization, role and phone number
- Your authority, if any: for example agent under an advance health care directive, conservator of the person, or trustee. A financial power of attorney or trusteeship alone usually doesn’t include health care decisions.
- Where the client lives and the type of setting
- The health care decision-maker’s name, phone number and preferred language
- The client’s physician and how to reach that office
- Whether hospice has already been discussed with the doctor or family
- Copies of an advance directive, POLST or conservatorship letters, if available (sent securely)
What updates you can expect
What we can share depends on your role and on the written permission we have. That’s how we protect every patient’s privacy.
- If you’re the client’s legal health care representative (for example agent or conservator of the person), you are part of care planning, and we keep you informed the way we keep any decision-maker informed.
- If you referred the client but aren’t the decision-maker, with appropriate authorization, we keep the referring professional informed.
- Without authorization, we can confirm that we received your call, and nothing more.
How to reach us
Call our 24/7 line at (714) 844-7777. Open 24 hours, 7 days a week. General email: care@maxcarehospice.com (please don’t send patient information by email).
Education, never payment
Ask us about in-service education for your team. We never pay, give gifts or offer anything of value for referrals, and a referral creates no obligation. The patient or their representative always chooses the hospice.
