Vietnamese American family-owned. Our full Vietnamese-speaking care team is here from your first call: call (714) 844-7777.
There are two ways to start. Choose the one that fits you.
For families: Ask our team about hospice
You can call us yourself. You don’t need a referral to ask questions. We’ll listen, help you think through whether hospice may fit, and work with your loved one’s doctor, because a physician still has to certify eligibility before hospice care can begin. Calling is a conversation, not a commitment.

For physicians, case managers and facility staff: Make a referral
- Phone: call our 24/7 line at (714) 844-7777. Open 24 hours, 7 days a week.
- Records: call our 24/7 line at (714) 844-7777 and we’ll tell you how to send records securely. Please never send records through a website form or regular email.
- What helps: the patient’s location and care setting, the decision-maker’s contact details and preferred language, a physician order, the diagnosis and recent notes showing decline, the medication list, the face sheet and the code status/POLST. A phone call is enough to start. See the full list
- Updates: with appropriate authorization, we keep the referring professional informed.

What happens after you call
- You talk with our team. Tell us who you’re calling about, where they are staying and how to reach you. We’ll ask which language you prefer. Please don’t put medical details in any online form.
- We answer your questions. We explain what hospice includes, how the Medicare hospice benefit works and whether hospice may be a fit. Asking doesn’t sign you up for anything.
- We work with the doctor. With your permission, we contact your loved one’s doctor for records and an order for a hospice evaluation. A physician must certify eligibility.
- We arrange a visit. A member of our clinical team meets you at home or at the facility, at a time we agree on together, to talk through care and check eligibility.
- Your family decides. If your loved one qualifies and chooses hospice, we plan care with you. If hospice isn’t the right fit yet, we can point you to other help.

What to send
A referral can start with a phone call. Our intake team will tell you which documents they still need. The more of these we receive, the faster eligibility can be confirmed:
- Patient name, date of birth and phone number (send through a secure channel, not this website)
- Signed physician order or referral for hospice evaluation
- Primary terminal diagnosis and relevant secondary or comorbid conditions
- Recent history & physical and progress notes showing decline
- Medication list and allergies
- Recent labs, imaging or other diagnostic results that support the prognosis
- Functional status: PPS or Karnofsky score, ADL dependence, FAST stage for dementia
- Evidence of decline: weight loss, recurrent infections, ER visits or hospitalizations in the past 6 months
- Insurance information (Medicare or other) and face sheet
- Advance directive, POLST and code status, if available
- Attending physician who will follow the patient (yes/no), plus contact information
- Caregiver or decision-maker name and phone; preferred language (we have Vietnamese-speaking staff)
- Care setting: home, assisted living, board and care, or skilled nursing facility
Hospice eligibility documentation checklist
- Physician certifies a terminal illness with a prognosis of 6 months or less if the disease runs its normal course
- Patient or representative chooses comfort-focused care over curative treatment for the terminal illness
- Clinical findings support the prognosis, for example declining PPS, weight loss, increasing ADL dependence or frequent hospitalizations
- Disease-specific indicators are documented (see our hospice eligibility guidelines)
- Face-to-face encounter documented when required for recertification
