What Hospice Means for Your Family
Hospice is comfort-focused care for people with a serious illness when the goal shifts from cure to quality of life. Our nurses manage pain and symptoms, our aides help with personal care, and we teach family caregivers what to expect. Patients can stop hospice at any time. You don’t need to wait until the last few days: hospice is comfort and family support, not giving up, and care can begin as soon as your loved one qualifies.
Hospitals and Care Settings in Rancho Cucamonga
Patients in Rancho Cucamonga are often discharged from nearby hospitals such as San Antonio Regional Hospital in Upland, Kaiser Permanente Fontana Medical Center, Kaiser Permanente Ontario Medical Center and Pomona Valley Hospital Medical Center. We work with discharge planners so care at home can start without long gaps. We care for patients wherever they live: private homes, assisted living, board-and-care homes and skilled nursing facilities.
Support for Family Caregivers
Many Rancho Cucamonga families care for a parent at home while working full time. Our aides help with bathing and personal care, our nurses visit on a regular schedule and are on call 24/7, and our social worker and chaplain support the whole family, including relatives who live far away.
Paying for Hospice
For most patients, hospice is covered by the Medicare Hospice Benefit under Medicare Part A. Many private plans also cover hospice. A patient generally qualifies when a doctor certifies a life expectancy of six months or less if the illness follows its usual course.
Languages We Speak
Our nurses (RN/LVN), aides (CHHA), administrator and liaison speak Vietnamese, so elders and relatives can ask questions and make decisions in the language they’re most comfortable with.
