Fountain Valley has two hospitals within its own city limits, UCI Health – Fountain Valley on Euclid Street and MemorialCare Orange Coast Medical Center on Talbert Avenue. So for many families here, the hospice conversation starts in a hospital room, often with little warning. If your family is facing that conversation now, here’s what to expect.
Maxcare Hospice is a Vietnamese American family-owned hospice. Our office is right next door in Westminster, and we’ve been caring for Orange County families since 2011, including many in the neighborhoods around Mile Square Regional Park.

Going home with hospice
When a doctor says hospice may be the right next step, you can ask the case manager to contact the hospice your family prefers. We then coordinate with the hospital team and your loved one’s doctor. Medical equipment, such as a hospital bed, can be part of the hospice plan of care, and our team helps make sure the home is ready.
We mention these hospitals only because many of our families come from them. Maxcare isn’t affiliated with either.

The Hospice Election Statement
Medicare requires a signed Hospice Election Statement before hospice services start. It names the hospice providing care, the start date, and the doctor your loved one chooses as their attending doctor, and it explains important information about hospice services. We’ll go through it with you line by line. You can also ask for an addendum listing any items, services or drugs the hospice has decided aren’t related to the terminal illness. (medicare.gov)
The first days at home
Every family is different, but early visits usually include:
- a nurse visit to check comfort, medicines and equipment;
- a plan for pain and other symptoms, built with your family;
- a visit from the social worker and, if you’d like, a chaplain;
- clear instructions about who to call at night. A live person answers our phone 24/7.
Fountain Valley resources
The Center at Founders Village, Fountain Valley’s senior and community center on Bushard Street, offers support groups and resource referrals, including grief support, according to the City of Fountain Valley. It can be a steady place for a spouse or caregiver during and after hospice.

Many Fountain Valley parents have children in other states. We can schedule calls so out-of-town relatives hear updates directly from the nurse or social worker, with your loved one’s permission. Vietnamese-speaking hospice care in Fountain Valley
Fountain Valley sits right next to Westminster, and many families here are part of the Little Saigon community. Maxcare Hospice’s nurses (RN and LVN), aides, administrators and office staff speak Vietnamese, so parents and grandparents can tell the nurse how they feel in their own words. We can walk the whole family through the Medicare hospice benefit, medicines and what to expect, in Vietnamese or English. If a loved one is coming home from the hospital, we can coordinate with the discharge planner when the family agrees.
